This incident occurred within the Fairfield City limits?
This did not occur on a state freeway?
This is not a theft report for a firearm, Motor vehicle, or a License Plate?

If you answered yes to all of the above questions, you are ready to file your report online. Please make sure to turn off your pop-up blocking software before filing the report.

Upon completion of this report, you will receive a temporary case number until report is reviewed. After case is reviewed, you will receive an official case number via email.
