Please read the following before filling out and submitting a request for a copy of a Police Report.

You must be a party involved in the report, or the legal representative of an involved party (authorization is required, and must be attached to your request).

You can mail your request along with a check or money order for $5.00 made payable to the " City of Fairfield" to:

You can also make your request in person at the police department. Please note, you will not be given a copy of your report then as we have 10 days by law to respond to your request.
